Essentia Health South University Clinic

Vaccinations will also be available and are open to all age groups.
A sports physical helps determine if it is safe for your child to participate in a particular sport. Most states, including North Dakota and Minnesota, require student-athletes to have a sports physical before starting a new sport (including practice) or before starting a new competitive season. Sports physicals are valid for one year in North Dakota and three years in Minnesota.
Prior to your appointment, please complete and sign the appropriate form available online at ndhsaa.com (North Dakota) and mshsl.org (Minnesota) and bring a copy with you. As of July 15, this form can also be completed via MyChart with an active account.
A sports physical can be done on its own or as part of a regular medical exam. Check with your insurance plan to determine coverage. A physical is an opportunity to review your child’s overall health, including growth, cognitive and social development, disease prevention, and mental health. We recommend annual physicals.
A sports physical is $57 and will not be billed through insurance. Athletes are seen on a first-come, first-served basis. Appointments are recommended but not required. Call (701) 364-8900 to schedule an appointment. You do not have to be an Essentia patient to be examined.
